

We currently accept the following insurance providers:
* Insurance verification is required prior to your appointment to confirm In-Network status for your plan.
* Payment plans may be available. We also accept CareCredit.
* Important: We do not accept all insurance plans.

□ Online Appointments: All online appointments must be verbally confirmed by our staff before you will be seen.
□ Doctor's Order & Insurance: A valid Doctor's Order is required for any Diagnostic exam. Please verify your insurance with our staff before your appointment. Failure to do so may result in delays or rescheduling.
□ Children in the Office: Children must be accompanied by an adult in the waiting area. Those under 5 must have an additional adult present inside the exam room. For Diagnostic Exams, children under age 7 are not allowed in the exam room unless they are the patient.
Our staff is not responsible for supervising children in the waiting area.
❗ No-Show Policy: A $10 No-Show fee is required at the time of rescheduling if you miss a scheduled appointment without canceling. Avoid this by canceling at least 4 hours in advance.
□ Payment in full is required to reschedule any previous No-Show appointment.
□ Self-Pay Pricing: Covers the ultrasound exam and final Radiologist/Cardiologist report. These prices are for self-pay patients only and do not apply to insurance billing.
Effective date for current pricing: January 5, 2023. Outdated forms with old pricing will not be honored.
□ Requesting Your Images: Let us know if you would like a copy of your ultrasound on a CD. Please allow 24–48 hours after your appointment for processing. We do not mail out CDs.
□ ID & Insurance: All patients must bring a valid photo ID and their current insurance card to the appointment. This is necessary for verification and billing.
□️ Military & Veterans: Show your Military or Veteran ID for a 10% discount. Note: A DD-214 is not accepted as valid ID (per Christopher Merkel, Owner/Veteran).
□ Checks Not Accepted: Please come prepared with an alternate form of payment. We do not accept personal or business checks.
