A valid Doctor's Order is REQUIRED for your Diagnostic exam(s) prior to being seen at your scheduled appointment.
NO Show fee of $10 will be due, at the Time of Reschedule, for those that NO-SHOW for their previously scheduled appointment. In order to avoid a NO-SHOW fee you must CANCEL your appointment at least 4 hours prior to your scheduled appointment time.
Payment in-full is required to schedule/reschedule from a previous NO SHOW APPOINTMENT.
Self-Pay Prices above cover the ultrasound exam and the Final Report from the Radiologist/Cardiologist. Self Pay Prices are NOT insurance prices. Self Pay Prices are effective 1-5-2023. Outdated order forms' Self Pay Prices are invalid.
If you'd like a copy of your ultrasound exam(s) on a CD please notify us. Please allow 24-48 after the exam/request for a CD. We do NOT mail out CDs.
Children under the age of 7 are not allowed in the exam room during any Diagnostic Exam, unless the child is the patient. Children waiting in the reception area must be accompanied by a parent or guardian, as our staff is not responsible for their supervision.
Patients having a diagnostic exam must bring Picture ID to the appointment. Military & Veteran Picture IDs are required for a 10% discount. NOTE: DD-214 is NOT considered valid form of ID and will NOT be accepted, per Christopher Merkel/Owner/Veteran.
We require the patient bring current insurance card and valid ID with them the appointment, for insurance verification and claims billing when using insurances benefits.
